Health Evaluation Procedures
Our evaluations are a multi-step process. We will do our best to take you through the process as smoothly as possible. There are roughly four steps to the evaluation process:
- Blood and Urine Sample
- Online Surveys
- Testing Day Appointment
- Follow-up Appointment

Blood and Urine Collection Instructions
If you receive a bill for the blood and urine collection, do not pay the bill. University of Colorado Hospital is supposed to bill us directly. Contact Tiffany Lipsey immediately at email@example.com.
One Week Before Your Appointment
Health History Survey
- In your confirmation email, we will include individualized links to health history surveys.
- Please complete and submit one week prior to your scheduled appointment.
*Please know your information is kept confidential. We will ask your permission to use your data for future heart disease prevention research.
Longmont Department of Public Safety would also like you to complete a Nutrition Survey.
- The four-day food log is optional.
- We would appreciate submitting these to us electronically if possible.

Testing Day Preparation Instructions
Because our evaluation is so comprehensive, there are a lot of steps involved. Take a moment to watch the video to prep you for what will happen on your testing day.
When Do I Get My Results?
At Your Follow-Up Visit
The follow-up visit is an individual consulting session to discuss your test results in detail.
We can schedule your follow up visit at CSU or at your department depending on the agreement with your department.
One of our trained staff members will give you feedback on your exam results.

Expect this visit to take one hour.
