Human Performance Clinical Research Laboratory Maps and Directions

The UCHealth system uses a patient data management system called EPIC. When your blood and urine samples are collected and analyzed, the results are stored in EPIC. This allows you to log into a patient portal and see the results. It also allows care providers who use EPIC to access your results through Care Everywhere, which is a platform for care providers to coordinate care across specialties, reduce/eliminate replication of tests, and improve care. This is advantageous to you because if your care provider uses EPIC, the results are visible for treatment, payment, and healthcare operations (TPO) pertaining to the specific treatment in question. Because your blood and urine results may be used for treatment, this access does not require an authorization for release.

However, if you do not wish to allow this access, you may request a restriction. In order to do this, you must complete the Request for Restriction on Uses and Disclosures of Protected Health Information by UCHealth form with a copy of your photo identification (such as a driver’s license) to:

The request will be reviewed by UCHealth Compliance, and a response of approval or denial will be sent to you in writing within 60 days.

• I-25/ Hwy 87 north to Prospect Rd, Exit #268
• Travel west (left) onto Prospect Rd to the intersection of Prospect Rd & South Shields St. (∼5 miles).
• Turn north (right) onto South Shields St.
• Continue north to the intersection of South Shields St. and West Elizabeth St. (∼1/2 mile).
• Turn east (right) onto W. Elizabeth St. into the Moby Auditorium/Gymnasium Parking Lot.
